What this test measures
The Testosterone Total test measures the total amount of testosterone circulating in your blood — both protein-bound testosterone (attached to SHBG and albumin) and the small fraction of “free” testosterone that’s biologically active. It is the standard first-line test for evaluating testosterone status in both men and women.
Who should take it
- Men: low libido, erectile dysfunction, unexplained fatigue, loss of muscle mass, increased body fat, mood changes, infertility, or are on testosterone replacement therapy (TRT)
- Women: irregular periods, excess facial or body hair (hirsutism), persistent acne, difficulty conceiving, or suspected polycystic ovary syndrome (PCOS)
- Anyone: unexplained osteoporosis, brain fog, or as part of a hormone health checkup

Normal reference ranges vary by lab, age, and sex. As a general guide: Adult males 240-950 ng/dL (8.3-33 nmol/L); Adult females 8-60 ng/dL (0.3-2.1 nmol/L). A single low reading isn’t a diagnosis — your doctor will want to confirm with a repeat morning sample and may add LH, FSH, and SHBG to investigate the cause.
